The incident took place as Karishma was being transferred from the delivery ward to the ground floor in a hospital lift. During the descent, the lift cable snapped, causing the lift to crash and resulting in fatal head and neck injuries to Karishma. The two hospital employees who were with her in the lift also sustained injuries. They were rescued after a 45-minute struggle to break open the lift door.
